The Cape Cod Metropolitan Planning Organization (MPO) was designated following the enactment of the federal transportation legislation entitled: The Intermodal Surface Transportation Efficiency Act of 1991 (ISTEA), and the 1990 United States Census of Population.

The meetings of the Cape Cod Metropolitan Planning Organization (MPO) are typically held in Barnstable County and are scheduled as necessary. The MPO meeting agendas are posted in the Barnstable County offices, and are mailed to individuals, businesses, and anyone who requests to be on the "Transportation" mailing list. At both the Cape Cod Joint Transportation Committee and MPO meetings, the regional transportation planning information and documents are presented. All are welcome to these public meetings on transportation planning. One of the documents that is typically updated each year is the Cape Cod Transportation Improvement Program, or TIP. The MPO votes on endorsement of the TIP. With TIP endorsement and all necessary project permits and approvals, transit and/or highway project implementation moves forward.

The Cape Cod Metropolitan Planning Organization (MPO) voted to restructure itself on May 19, 2005, and now includes more local members. As of November 2005, the MPO now includes nine voting members and six Ex-Officio members. The current nine voting members are:
